COVID-19 studio safety plan & release form

 

We are happy to be providing in person classes. Please read below to find out what we are doing to reduce COVID-19 transmission and how that will impact classes.

AERO STUDIO POLICY, COVID-19 & SAFETY AGREEMENT

ATTENDANCE, CANCELLATIONS & NO-SHOWS

Please make sure you allow plenty of time to arrive at the studio as we often have trains and Portland traffic is unpredictable. Students must arrive on time for class in order to attend.

For client safety, studio doors are locked 5 minutes after the start of class. Students who arrive after the door is locked may not be permitted to enter the building.

The AERO Space does not provide any refunds or makeups. If a client is unable to attend class they must cancel in our scheduling software.

Early cancellations for drop in classes (at least 48 hours prior to the start of class) will receive a credit back in our registration software for future use of that class.

Late cancellations (less than 48 hours prior to the start of class) will not receive a class credit. is unable to attend class they must cancel in registration software prior to the start of class.

We do not provide any cancellations, makeups or refunds for any Series classes.

No shows will be charged $5 per missed class.

No refunds will be provided for used or unused memberships, packages or classes. If a client is ill, diagnosed or has been exposed to Covid-19, the studio will extend the expiration date of a class package up to 14 days. Extensions and reasonable special arrangements may be made for extenuating circumstances.

COVID-19 STUDIO SAFETY

AERO is committed to providing a safe workspace for our team, clients and members. Due to the COVID-19 pandemic, recommendations and guidelines by the CDC, State of Oregon and HIPAA, we have increased the cleaning and safety protocols in our studio. This document outlines these protocols and precautionary measures we are taking to reduce risk of COVID-19 transmission in our studio.

Every student and member is expected to take part in implementing and complying with these measures so that we can help prevent the spread of and assist in contact tracing for COVID-19.

SELF SYMPTOM SCREENING

All clients, members and instructors should self-monitor for these symptoms. Anyone showing any COVID symptoms should not attend classes and follow CDC guidelines for when it is safe to return to normal activities.

  • Cough

  • Shortness of breath or difficulty breathing

  • Fever

  • Chills

  • Muscle pain

  • Loss of taste or smell

  • Sore throat

  • Headache

HYGIENE, SANITATION AND PHYSICAL DISTANCING

All clients must implement good hygiene and infection control practices. Clients are instructed to wash their hands for at least 20 seconds with soap and water prior to the start of class and after using the restroom.

Each client will be asked to disinfect their equipment prior to and following each class using 70%+ alcohol at the recommendations from the CDC.

Only clients who have pre-registered lessons or classes will be allowed to enter the studios.

Everyone must adhere to the State/County’s guidelines of wearing face covering. Disposable masks are available for use in the studio.

RELEASE AND HARM HOLD AGREEMENT

In consideration for use of The AERO Space exercise and dance facilities at Portland Cement Studios, 111 SE Madison and 7202 NE Glisan St as a tenant, guest, dance instructor or student, I agree to:

1) Assume all risks of injury, death or damages as may occur during the course of use of the facilities and/or equipment, or as may be directly related to such use.

2) Release the business owner, The Aero Space, and building owner, Portland Cement Building LLC and Portland Cement Studios and its Manager, Rachelle Rhoe, and Ashley Madison, all of their respective employees or independent contractors from all claims that I might have against owner and manager for injury, death or damages resulting from my use of facilities and/or equipment.

3) Hold the business owner, building owner, manager and tenants harmless from any claims for injury, death, or damages resulting from my use of facilities and/or equipment.

I am aware that a risk of injury or complication of my health/physical condition is present with exercise and dance, and assume all risks of participation. I agree to use the facilities and equipment properly and to the best of my knowledge and ability. I am physically sound, having medical approval to participate in activities of this nature.

In consideration of the privilege granted me of facilities and/or equipment usage, I, intending to be legally bound, hereby for myself, my heirs, executors, administrators and assignees, waive and release any and all rights and claims I may have against the owner, Portland Cement Building LLC and Portland Cement Studios, together with its respective officers, agents, employees, representatives, successors, managers, tenants, or assignees for any and all injuries, ailments, or other consequences suffered by me during use of the facilities and/or equipment.

PHOTOGRAPHS

The AERO Space will often take photographs of clients and staff, or photographs in which the clients/members may be involved with others for the purpose of promoting The AERO Space. Staff will always ask permission for use of photographs. To opt out, simply notify staff. Failure to exercise this option, releases and discharges The AERO Space from any and all claims arising out of the use of photographs, or any right that the student or member may have.

The undersigned represents that he/she has read, understands, and is voluntarily signing this release and hold harmless Agreement and, further, represent that no verbal statements have been made to the undersigned to induce him/her to sign this Agreement.

ACKNOWLEDGEMENT

I acknowledge I have read, agree to AERO STUDIO POLICY, COVID-19 & SAFETY AGREEMENT and will follow all policies and procedures contained within the plan.
